Boulo Solutions is sourcing candidates for a non profit in Birmingham, AL (The Parkinson's Association of Alabama) looking to hire a full time, remote mainly, Care Navigator to join their team. The Care Navigator will serve as the key person who connects Alabamians with the non profit's mission and their families to services and resources. This position provides education and support to healthcare professionals and the public to further the mission of improving quality of life for those affected by the disease through awareness, education, advocacy, and care.
Employment Type: Full-Time
Flexibility: Remote in Birmingham, Alabama (wlll travel around for meetings)
Pay: $45,000 (will be a temp to permanent hire after 90 days)
Benefits: Worker's comp, one week paid vacation after 6months and 2 weeks after 1 year. 7 Paid Holidays.
Key Responsibilities:
1. Support Services:
- Operate a toll-free support line and manage online inquiries through the website.
- Document all calls and maintain confidential records of interactions.
- Provide non-medical advice and support to patients, caregivers, and family members.
2. Education and Resources:
- Equip patients and caregivers with information on treatment and therapy options.
- Collaborate with healthcare professionals to develop relationships and establish referral networks.
- Work with Marketing to create content for monthly newsletters, blogs, and the website.
Organize virtual Zoom Support Groups:
- Living with Parkinsons (1 st Wednesday of each month)
- Caregiving Support Group (2 nd Tuesday of each month)
- Parkinson’s and Dementia Caregiver Support Group (2 nd Sunday of each month)
3. Community Engagement:
- Establish and nurture relationships with PTs, Neurologists, and Movement-Based Exercise Programs.
- Represent PAA at community events, mini-symposiums, and exhibitions.
- Assist with Support Group start-ups, including North Alabama’s New Care Navigator initiative.
4. Annual Parkinson’s Patient Symposium:
- Secure venue, speakers, and build the agenda for the annual symposium.
- Promote awareness of the event and manage logistics.
- Distribute PAA flyers and conduct surveys to gather feedback.
5. Funding and Grants:
- Assist in identifying and applying for funding opportunities from pharmaceutical companies and grants
- Explore and establish relationships with pharmaceuticals and other potential sponsors.
6. Administrative Duties
- Provide staff support to the Mission Committee, including preparing agendas and informal meeting notes.
- Maintain a monthly “plan on a page” update to track progress and activities.
- Work closely with the Mission Board Chair.
7. Volunteer Network:
- Establish and manage a volunteer network across the state (South, Birmingham, Central, and North Alabama).
- Coordinate volunteer activities and utilize their support for various programs and events.
Qualifications:

Experience working with individuals with chronic illnesses, preferably Parkinson’s disease.

Strong interpersonal, communication, and organizational skills.

Knowledge of Parkinson’s disease management, treatment options, and community resources.

Proficiency in Microsoft Office and online communication platforms.
Preferred Qualifications:

Healthcare background or knowledge of healthcare systems preferred.

Experience in patient navigation, support group facilitation, or community outreach roles.
